Home-Based Chronic Disease Management Service for the Elderly (CDME) launched
More than 200 volunteers have joined the HKRC’s Chronic Disease Management Service for the Elderly (CDME) project. About 470 elderly in need are expected to be benefited from the brand new project.
The Community Care Service Department of HKRC officially launched the CDME on 1 6 Aug 2014. Chairman of Community Care Service Management Committee Ms Virginia Chan and five committee members (Mr. Stephen Ma, Mr. John Ng, Dr Liu Kin Wah, Mrs. Anna Wong, Mrs. Holly Wong), together with other attending guests, shared the joy of the nearly 200 volunteers present.
CDME project targets to serve the singleton elderly or elderly couples in Sham Shui Po, Shatin and Tuen Mun who suffer from diabetes or hypertension and rely on public allowances. Each elderly will receive the service for 12 months to strengthen their self-management skills on chronic diseases. The 3-year project is expected to serve about 300 elderly.
