The “Home-Based Chronic Disease Management Service for the Elderly” (CDME) aims to serve needy or singleton elderly with hypertension and diabetes, who lack community support. HKRC staff and health professionals will visit the elderly at their home to assess each individual’s health conditions and develop treatment plans for each. Our volunteers will then visit the elderly regularly to follow up with the plans, aiming to empower the elderly with positive attitudes and self-managing skills in dealing with chronic diseases. We will also help establish and strengthen their community support network.

The project is now recruiting volunteers. Anyone aged 15 or above with passion and caring spirit, who is willing to provide home-visit and supporting service to elderly are welcome to join. The HKRC shall provide 18-hour training to shortlisted volunteers.
