Chronic Disease Management Service for Elderly


The Hong Kong Red Cross (HKRC) has launched the brand new “Home-Based Chronic Disease Management Service for the Elderly” (CDME) in June this year. Within 3 years, the project will serve at least 470 needy or singleton elderly with hypertension and diabetes, who lack community support. We aim to enhance their self-managing skills for chronic diseases, as well as to help strengthening the societal support network for them. 


The Community Care Service Department of HKRC will work closely with District Elderly Community Centres under the CDME project to be piloted in the districts of Sham Shui-Po, Shatin and Tuen Mun. Adopting the “case management approach”, the project will be run on “door-to-door” basis. HKRC staff and health professionals will visit the elderly at their home to assess each individual’s health conditions and develop treatment plans for each. Our volunteers will then visit the elderly regularly to follow up with the plans, aiming to empower the elderly with positive attitudes and self-managing skills in dealing with chronic diseases. We will also help establish and strengthen their community support network.


With an aging population, and life expectancy on the rise, Hong Kong has been facing more and more elderly patients with hypertension and diabetes. We hope that with the support of enthusiastic organizations in the community, our volunteers under the CDME project will be able to offer support to more senior citizens in need.
